Established in 1974, Area Five Agency on Aging and Community Services is an independent not-for-profit organization dedicated to serving the needs of the elderly, the disabled and the disadvantaged members of North Central Indiana.
Nutrition Site Manager – Wabash (part-time)
Facilitates the serving of the noon meal program. Duties may include: calling in meal reservations in a timely manner, maintaining appropriate documentation and money handling procedures, following proper portion control servings, etc. Must be sensitive to the needs of older adults & have a valid driver’s license.
Outreach & Enrollment Specialist – Logansport (part-time)
Assists individuals and families requiring the assistance of a social service agency to connect them to and determine eligibility for community resources, including health insurance coverage. Provides support to the other program areas within the Agency, as needed. This position is office based and requires the individual to respond to requests for information over the phone. This position requires frequent outreach in the community. Ability to define problems, collect data, establish facts, draw valid conclusions and make recommendations is a must. This requires the ability to maintain current program and resource knowledge and integrate this information into resolutions for callers. Education and/or Experience: High school diploma or equivalent.
Vaccine Equity and Access Program (VEAP) Project Coordinator – Logansport (part-time)
VEAP is a national program administered at a local level to facilitate information about and access to vaccines, specifically Influenza and Covid-19. This position is responsible for daily program activities, community outreach, communication in both English and Spanish to city residents, information distribution, equipping community leaders with knowledge, cultivating partnerships with local health department and hospital, planning community events, reporting activities and outcomes to Program Manager. Other related duties as assigned. This position is ideal for someone who is comfortable interacting with the public, who develops and carries out creative engagement strategies for outreach to underserved, low-income, and racial and ethnic minority populations. Flexible work schedule. Requirements: High School Diploma or Equivalent. Valid Driver’s License and reliable transportation. Bilingual ability (Spanish/ English) preferred.
Head Start Lead Teacher – Logansport (full-time)
37.5 hours per week. Responsible for planning and executing all component services for Head Start children and families according to the National Head Start Performance Standards and the local component plans. Associate Degree required, pay depends on experience and education.
Meal Deliverer – Rochester (part-time)
Assists with packaging and delivery of meals to specific individuals on assigned route. Cleans caddies after route delivery & helps, if needed, with cleaning the meal site. Observes while delivering for any unusual circumstances or changes in normal routine at participant homes and reports to site manager any observance that may be a matter of concern. Must be sensitive to the needs of older adults & have a valid driver’s license.
Substitute Nutrition Site Manager – Wabash (part-time)
Substitutes for our current Nutrition Site Manager as needed by facilitating the serving of the noon meal program. Duties may include: calling in meal reservations in a timely manner, maintaining appropriate documentation and money handling procedures, following proper portion control servings, etc. Must be sensitive to the needs of older adults & have a valid driver’s license.
Options Counselor – Logansport (full-time)
Counsels and aids individuals and families requiring information and assistance. Provides direct Options Counseling by assessing and analyzing client requests and/or needs; explores problem resolution alternatives. Completes insurance application assistance and understanding, facilitate referrals to community and agency programs, and intervention as appropriate. Qualifications: A Qualified Intellectual Disability Professional who meets the requirements at 42 CFR 483.430, or a registered nurse with one year’s experience in human services, or a bachelor’s degree in social work, psychology, sociology, counseling, gerontology, nursing, health and human services, or a bachelor’s degree in any field with a minimum of two years of full-time, direct-service experience with the elderly or disabled (this experience includes assessment, care plan development, and monitoring), or a master’s degree in a related field may substitute for the required experience.
Kitchen Assistant – Logansport (part-time)
The Area Five Agency on Aging and Community Services is looking an enthusiastic employee who is interested in food production. This is an opportunity to be a part of a great culinary team who serves seniors in our six county service area. Employee would be assisting with prepping, preparing, and packaging nutritious meals in the Logansport Culinary Facility which will be served at our meal sites. Position is part time up to 20 hours per week starting as soon as possible. Desirable employee will have a positive attitude, work well with others, be able to lift up to 40 pounds, and work mornings.
Substitute Kitchen Driver – Logansport (part-time)
Substitutes for the Kitchen Driver position as needed. Duties include: delivering meals to our Miami and Wabash County meal sites and assisting in the kitchen as needed. Must be able to lift 50 pounds, have a valid driver’s license, and be willing to learn how to operate the heated/refrigerated delivery truck mechanism.
Kitchen Driver – Logansport (part-time)
Part-time, Monday through Friday position, with morning hours. Duties include: delivering meals to our Miami and Wabash County meal sites and assisting in the kitchen as needed. Must be able to lift 50 pounds, have a valid driver’s license, and be willing to learn how to operate the heated/refrigerated delivery truck mechanism.
Case Manager – Miami & Howard Counties (full-time)
Perform In-home visits to determine needs and eligibility of individuals. Set up and monitor services on an ongoing basis. Work with many different agencies to ensure client’s safety and well-being is met. Must have reliable transportation with valid driver’s license and insurance. Computer skills, communication skills, and knowledge of community agency programs are helpful. Education: Requires a Bachelor’s degree (4 yr. degree) in Social Work, Psychology, Counseling, Gerontology, Nursing, or Health & Human Services; OR Bachelor’s degree (4 yr. degree) in any field with a minimum of two years full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring; OR Registered nurse; OR A Master’s degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services may substitute for the required minimum of two full time direct services experiences; OR An Associate’s degree in nursing; OR An Associate’s degree in any field with a minimum of four year full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring.
Facilities Maintenance Support Worker – Logansport (full-time)
Maintains and repairs physical structures of buildings and maintains grounds. Receives written work orders or verbal instructions from supervisor to perform structural repairs to masonry, woodwork, and furnishings of buildings. Performs grounds maintenance activities such as mowing grass, weed removal and disposal, trimming shrubs and trees, disposing of leaves and refuse. Moves furniture and files and unloads and stores supplies to assist other departments as requested. Repairs drywall and plasters walls, paints interior and exterior walls and trim; hangs wall paper and other finish material. Operates snow removal equipment to maintain parking lots and sidewalks. Maintains and repairs buildings' plumbing and electrical systems, including replacing worn or defective parts such as switches and fuses. Replaces worn or damaged parts such as hoses, wiring, belts, in machines and equipment. Performs basic carpentry work using hand and small power tools. Maintains tools and equipment in safe working condition; understands purpose and limits of all tools and equipment used. Assembles/dis-assembles furniture and equipment as required. Assists in maintaining safety & security of facilities and staff by monitoring the fire extinguishers and exit signs; helps maintain MSDS files, and transportation and storage of hazardous materials. Assist Facility Manager with Vehicle maintenance, repairs, and recording of such maintenance on the appropriate logs. Education: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Must maintain a valid driver's license and carry vehicle insurance.
Family Support Specialist –Miami/Wabash Counties (full-time)
Healthy Families is a home visitation program in which you will apply working knowledge of parent-child interaction, child development, safety issues, and the dynamics of child abuse and neglect to help enrolled families improve parenting skills and reduce family stress. Must be able to accurately prepare reports, documentation & data entry, communicate effectively, handle crisis situations in a calm manner, type & have computer knowledge, be organized and efficiently manage time. College degree preferred and/or one to three years’ experience or training working with infants and young children and in working with parents. Must have a valid driver’s license, reliable transportation and proof of car insurance.
EAP Client Service Specialist - 1 Cass County / 2 Howard County / 1 Miami County / 1 Tipton County (part-time)
No supervisory responsibilities. Bi-lingual applicants are preferred for Cass positions; however not exclusively. Individual is responsible for taking EAP applications, data entry, providing Energy Education and secretarial duties as needed. Work with clients and negotiate with utility vendors. Individual must have interpersonal skills which allow for effective communication with clients and utility vendors, maintain good organizational skills, and are able to calculate income efficiently. Some travel may be required. Other duties as assigned and/or necessary to get the required jobs done.
Head Start Teacher Assistant – Logansport (full-time)
37.5 hours per week. Assists the Teacher/Family Facilitator in planning and executing all component services for Head Start children and families according to the National Head Start Performance Standards and the local component plans. Child Development Associate Credential (CDA) required, Area Five is willing to assist with the cost of obtaining CDA.
