Current openings:

Community Health Worker Program Manager – Peru (Full-Time)

The CHW Program Manager is responsible for launching, coordinating, and improving Area Five's Health Transitions/Post-ER Preventive Care Program. The Lead turns the program plan into daily operations by building referral pathways, CHW workflows, partner relationships, documentation, data tracking, training, and performance reporting. The position requires a highly motivated, self-directed problem solver who can work independently and keep implementation moving.  High school diploma or equivalent required; 2 or more years of work experience in social services, associate degree, or bachelor's degree in a related field preferred.  Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area as required. CHW training/certification is not required at time of hire; however, the employee must obtain certification and complete all required program-specific training.

EAP Client Service Specialist – Kokomo (Part-Time)

This is a seasonal, part-time position. Individual is responsible for taking EAP applications, data entry, calculation of income, providing Energy Education and collaborative program duties as requested. Individual must have interpersonal skills which allow for effective communication between clients and utility vendors, while maintaining good organizational skills. Other duties as assigned and/or necessary to get the required jobs done.

Facilities Custodian – Logansport (part-time)

We are looking for a part-time custodian to work hours in the late afternoon & early evening. Job duties include keeping premises of main office building in clean and orderly condition by performing the following duties, as scheduled by supervisor.  Cleans & sanitizes rooms, hallways, lobbies, lounges, rest rooms, corridors and other work areas; Sweeps floors; Cleans rugs, carpets, upholstered furniture, draperies and mats; Dusts & sanitizes furniture and equipment. Polishes metalwork; Washes walls, ceiling, and woodwork. Washes windows, door panels, and sills; Empties wastebaskets; Transports trash and waste to disposal area.  High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

Meal Deliverer – Peru (part-time)

Assists with packaging and delivery of meals to specific individuals on assigned route.  Cleans caddies after route delivery & helps, if needed, with cleaning the meal site.  Observes while delivering for any unusual circumstances or changes in normal routine at participant homes and reports to site manager any observance that may be a matter of concern.  Must be sensitive to the needs of older adults & have a valid driver’s license.

Head Start Substitute Food Server / Teacher Assistant – Rochester (part-time, as needed)
Serves meals to children during Head Start program by performing the following duties:  Monitor temperature of food and serve food according to the serving sizes noted on the menu. Take dishes, plates, & silverware to classroom for teachers. Fill silverware containers after each meal and check on these during service to assure that there are plenty of napkins for each classroom. Fill and sanitize bottles and test the water, and clean up the kitchen equipment. Give Teacher's Assistants and or Teachers a 10-15 minute break if possible. Help staff clean up classroom by washing tables and emptying trash cans if possible. Kitchen must be cleaned daily including, sweeping floor, emptying trash, wiping counter, cart, shelves, cleaning warmer and refrigerator, and washing dishes. Check and report supply needs daily. Fill condiment containers as needed so they are full for the next time. Fill all food service requests from the teachers and teacher's assistants. (i.e. seconds on food, silverware, napkins, salt, condiments, cleaning tables, and emptying trash cans that include food.) Check to see if the teachers need anything during meals. High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
